Dear Team Member:
Our organization is committed to a strong and healthy work environment and you are the only ones who can tell us how we're doing. The Culture 20/20® is designed to learn about your experience as an employee at this organization and then to use the results to make adjustments where they are needed for the best overall employee engagement and organizational culture. Think of it as taking the organization to the doctor for a physical. We want to do a check-up on the organization and then prepare a plan for optimal health.
Your input on this survey will help us understand the strengths and weaknesses of our culture and take specific steps to address them, so please take time to complete the survey. Your honest opinions and constructive comments are very important to us, whether favorable or unfavorable.
To preserve confidentiality and anonymity, an external company, EDA, Inc. will consolidate the survey responses before providing them to our organization. Please note, that the comments you write in are scrambled and unidentifiable unless you identify yourself in your comment, so if you want to remain anonymous in your comments, just word them in a way that doesn't identify you as the writer.
